If you've spent any time on social media lately, you've probably heard that ashwagandha is the answer to menopause symptoms. But does the science support the hype?

Here's what the latest research tells us.
Ashwagandha (Withania somnifera) is an adaptogenic herb used in Ayurvedic medicine for centuries. Adaptogens are believed to help the body respond to physical and emotional stress by supporting the body's stress-response systems.
In recent years, researchers have begun studying whether ashwagandha may also help women during perimenopause and menopause.
Several randomized, double-blind, placebo-controlled trials have found encouraging results.
Women taking standardized ashwagandha root extract experienced improvements in:

✅ Sleep quality
✅ Stress and anxiety

✅ Psychological symptoms
✅ Physical symptoms
✅ Urogenital symptoms
✅ Overall menopause-related quality of life
Multiple studies also found significant improvements in Menopause Rating Scale (MRS) and Menopause-Specific Quality of Life (MENQOL) scores compared with placebo.

Several clinical trials found that women taking ashwagandha experienced:
• Increased estradiol (estrogen)
• Reduced FSH
• Reduced LH
These findings suggest that ashwagandha may influence the hypothalamic-pituitary-gonadal (HPG) axis, but researchers are still working to understand exactly how this occurs and what it means for long-term health.
Importantly, unlike findings in men, current studies have not shown meaningful increases in testosterone in women.
Emerging evidence suggests ashwagandha may also support bone health.
One 24-week randomized controlled trial reported improvements in markers of bone turnover, including reductions in bone resorption markers and increases in osteoprotegerin, a protein involved in maintaining bone strength.
These findings are encouraging, but larger studies measuring fracture risk and bone mineral density are still needed.
One of the biggest challenges is that there is no standardized dose.

Most positive studies lasted 8–12 weeks, although one menopause study followed women for 24 weeks.
Because products vary in extraction methods and concentrations, 500 mg from one brand may not be equivalent to 500 mg from another.
Ashwagandha is one of the more promising supplements being studied, but there are important limitations.
Current studies often include:

• Different extract formulations
• Different doses
• Different outcome measures
Because of these differences, researchers cannot yet determine:
Overall, short-term clinical trials suggest that standardized ashwagandha extracts are generally well tolerated, with most reported side effects being mild.
However, it's important to remember that "natural" does not always mean "risk-free."
Recent literature includes a case report describing hypothalamic-pituitary-adrenal (HPA) axis suppression and Cushingoid features in a postmenopausal woman taking approximately 950 mg daily for more than one year. While this represents a single case—not proof that the supplement causes this effect—it highlights why long-term safety requires further study.
Ashwagandha may also interact with certain medications and may not be appropriate for everyone, including people with thyroid disease, autoimmune conditions, or those who are pregnant or breastfeeding.

Ashwagandha is not a miracle supplement, but it is one of the most promising botanicals currently being studied for menopause.
The evidence suggests it may help reduce menopausal symptoms, improve stress and sleep, and support quality of life. It may also influence hormone regulation and bone turnover, but the research is still developing.
We need larger, longer-term studies using standardized formulations before clinicians can make firm recommendations about who should take it, at what dose, and for how long.
Until then, ashwagandha should be viewed as one potential tool—not a replacement for evidence-based menopause care, healthy lifestyle habits, or individualized medical treatment.
